Register with our practice
We are currently accepting new patients on to the Practice list and these will need to reside in a specified area. At present, we are taking on patients who live in Twickenham, Teddington and some parts of Hampton, Hanworth and Feltham.
Those wishing to register will need to complete an application form, a medical questionnaire and to provide proof of photo ID and two proof of address. You may also be asked to complete further health and alcohol questionnaires when you attend the surgery for your new patient check.

All new patients aged 16 and over are required to have a 10 minute New Patient Health Check with our healthcare assistant. Please call the surgery to arrange this, once you have completed your registration forms.
Please note, the medical questionnaire is for age 16 and over only. Anyone under this age just needs the application form completed and a proof of ID provided (Birth certificate will be accepted for children). For children under the age of five; Parents will be asked to complete a Health Visitor form on your first visit to the surgery.
